NPCBench: A New Benchmark for Multimodal Care-Pathway Reasoning in Nasopharyngeal Carcinoma

NPCBench is a clinical apprenticeship benchmark for nasopharyngeal carcinoma that evaluates AI models on integrated, longitudinal care-pathway reasoning rather than isolated questions. It includes 4,777 items and a 100-patient cohort with 417 scored tasks, testing evidence grounding and temporal coherence. Evaluations of 30 models reveal a composition gap: strong individual task performance does not guarantee jointly correct diagnostic and care answers. The benchmark aims to push medical AI beyond static QA toward workflow-oriented assessment.

MedCBR Targets Difficult Diagnostic Cases to Align Medical Vision-Language Models

The MedCBR research framework combines boundary-aware preference optimization with reinforcement learning guided by clinical rubrics. It focuses training on cases near a model’s capability limit and uses prediction entropy to gate rewards for answer correctness, anatomical grounding and reasoning fidelity. The authors report 61.2% average accuracy across six medical benchmarks, with the largest gains on challenging differential diagnosis tasks; these benchmark results do not establish clinical readiness.

Why it matters: The paper offers a concrete alignment approach for medical vision-language models that addresses both difficult diagnostic cases and reasoning quality beyond final-answer correctness.
